ANSA Nomination Form 1
All nominations to be completed in full. ANSA requires all submissions to be accompanied by a recent passport-size photograph of the nominee, certified copy of National ID for those over 16years of age, certified copy of Birth Certificate for those under16years of age, certified copy of passport, proof of all achievements and any extra information to validate the nomination. ANSA Judges Panel is obliged to retain materials submitted with the nomination. Please note that only achievements for the year under review are required and that associations can nominate a maximum of two persons per category.
